
How does it all work?
All clients with new or existing diagnoses who will need reinforced instructions to manage their conditions qualify for our services. The clients insurance will pay for all the necessary services, as well as the caretaker's time, at no additional cost to the client or family.
The first step toward receiving home health care is to obtain a physician's orders to recieve services in the home. Convergent Care will work with potential clients to develop a detailed care plan.
The next phase is planning. Convergent Care will complete an initial consultation with the client, during which we will assess the client's needs and develop a plan for care. Family members and other caregivers are encouraged to participate in this planning process to ensure consistent and comprehensive care.
When services begin, Convergent Care staff will implement the plan – following all physician orders – and keep the physician updated about the client's progress. The frequency and type of home health visits will vary depending on the client's needs. Some clients require daily care while others require only a short visit once or twice per week. All services are tailored to the client's needs.

What is Medicaid?
Medicaid offers benefits that Medicare doesn’t normally cover, like nursing home care and personal care services. People with Medicaid usually don’t pay anything for covered medical expenses but may owe a small co-payment for some items or services. Convergent Care has the ability to provide all of the services covered under Medicaid that pertain to home healthcare. Most of our clients have some form of Medicaid.

What is Medicare?
Medicare is federal health insurance for people 65 or older, and some people under 65 with certain disabilities or conditions. A federal agency called the Centers for Medicare & Medicaid Services runs Medicare. Because it’s a federal program, Medicare has set standards for costs and coverage. Convergent Care does NOT provide the services covered under the Medicare program, as they are generally performed by professionals in conjunction with the client's primary physician.
